D0 – preparation to start the analysis and emergency actions
D1 – appointing a cross-functional problem-solving team
D2 – precise definition and description of the problem: 5W2H, OK/NOK analysis, histogram, Pareto-Lorenz analysis, process flow diagram
D3 – implementation and verification of immediate actions
D4 – identification and confirmation of root causes: cause-and-effect diagram, 5 Why method, is/is-not analysis, brainstorming, cause of occurrence and cause of non-detection of the problem
